• Medical history
• Medications

MyChart offers PHS patients better access, convenience and control. It serves as a platform to make it easy for you to communicate with your physician and actively participate in managing your own care. With MyChart, you can:

• Change personal contact and billing information
• Email your physician to ask medical questions
• Manage appointments (schedule or cancel appointments, and view upcoming or past appointments)
• Renew prescriptions
